When returning equipment, please follow the existing steps:

1. If returning in the same box you received the equipment, please remove the label with your name and school.

2. Download and Print the following word document and attach it to the SIDE of the box.

3. Take the box(es) to the Front Office.

4. Print the following t-req form, fill in the appropriate blanks and email it to both of the following email addresses:
